Distributions of patient results from Fürst Medical Laboratory (March 2004) compared to NORIP distributions of reference values

Fürst Medical Laboratory, Oslo, Norway:

Origin of samples

- primarily from primary health care

Transport

- 75 % by car transport to lab.

- 16 % by post

- 9 % collected at lab.

Sample collection

- fasting (not been eating, drinking or smoking since midnight) for triglycerides (at least 12 h fasting) and glucose

- sample material: Serum from Vacutainer SST 6 mL gel sample tubes

- centrifugation: 1300g for 10 min within 2 hours after collection

Measurement system

- Roche Modular

Trueness: The laboratory have tested Roche Modular for all components using NFKK Reference Serum X and CAL and used the Excel spreadsheet for evaluation of bias. The result for CAL is shown together with the distribution(s) for each component:

Plots Comments
Clinical chemistry
Albumin 40-70 years, 18-40 and >70 years Brom Cresol Green method. Estimated bias is +1.4 g/L
Bilirubin   Sample must be protected against light.
Calcium   Uncorrected and corrected (estimated bias is +0.04 mmol/L).
Carbamide (urea) Female, male 18-49 years, male >=50 years Notice the difference in shape of Fürst and NORIP distributions for individuals > 50 years (see also uric acid).
Cholesterol 18-30 years and 30-50 years, >50 years
Creatininium, corr   At the moment (24/9-04) our results is multiplied with 0.88 to match former trueness from Roche. The distributions from Fürst presented here is divided with 0.88 and then subtracted 26 µmol/L to match latest recommendations from Roche to comply with traceability to reference method.
Glucose   Fasting from midnight before sample collection.
HDL-cholesterol
Iron   No iron medication within at least 24 h before sample collection.
LDL-cholesterol, Friedewalds formula 18-29 years and 30-49 years, >= 50 years Friedewalds formula: LDL-chol. = Cholesterol - HDL-chol. - triglycerides/2.2. Not calculated if triglycerides >4.00 mmol/L.
LDL-cholesterol, direct method 18-29 years and 30-49 years, >= 50 years Roche, enzymatic method without precipitation
Magnesium   No or light and brief stasis at sample collection.
Phosphate Female, male 18-50 years, male >=50 years No or light and brief stasis at sample collection.
Potassium   No or light and brief stasis at sample collection.
Protein
Sodium
TIBC   Measured as transferrin, factor 25.1 µmol/g
Triglycerides   Fasting at least 12 h
Urate (uric acid) Female, male Notice the difference in shape of Fürst and NORIP distributions for individuals >50 years. See also carbamide (urea).
ALP
ALT
Amylase
Amylase, pancreatic
AST
CK Female, male Sample must be protected against light. Instruction to patient:  The pasient should not have performed unusual heavy physical activity, been subject to physiotherapy or have had intramuscular injections the last 3 days before sample collection.
Gamma GT Female, male
LD   No or light and brief stasis at sample collection.
